The Responding to Unwell Service Users service from Unity Care provides rapid clinical support when someone’s health changes. Our trained clinicians can attend the home to assess the situation, carry out health checks, and help determine the most appropriate next steps.
What is this service?
This service provides on-call clinical assessment at home when a service user becomes unwell. A trained and qualified clinician attends the home to carry out a clinical examination and monitor key health indicators.
These assessments help build a clearer picture of the person’s condition and help guide decisions about whether further medical treatment or referrals may be required.

What checks may be carried out?
During a visit, clinicians may carry out a range of observations and health checks, which can include:
Blood pressure monitoring
Heart rate monitoring
Breathing rate assessment
Blood oxygen level checks
Blood glucose monitoring
Temperature checks
Where appropriate, additional monitoring such as cardiac monitoring or an ECG may also be performed.
